Unitedhealthcare Senior Care Options Plan
UnitedHealthcare SCO is a Coordinated Care plan with a Medicare contract and a contract with the Commonwealth of Massachusetts Medicaid program. Enrollment in the plan depends on the plans contract renewal with Medicare. This plan is a voluntary program that is available to anyone 65 and older who qualifies for MassHealth Standard and Original Medicare. If you have MassHealth Standard, but you do not qualify for Original Medicare, you may still be eligible to enroll in our MassHealth Senior Care Option plan and receive all of your MassHealth benefits through our SCO program.
If You Qualify Under A Magi Eligibility Group You Will Have To Provide Documents To Verify Eligibility If Necessary
- If you are applying for Medicaid through the Marketplace , you may attest to your household income for the upcoming year. If your income is different than the income found on the data matches, income documentation may need to be provided.

Medicare Part A Monthly Premiums And Penalties
Heres what you might expect to pay if you have not contributed towards Medicare through payroll taxes :
If you paid less than 39 quarters – $274.00 p/m.
If you paid less than 30 quarters – $499.00 p/m.
If you made no contributions towards Medicare – up to $499.00 p/m.
If you dont have premium-free Medicare Part A, you will have to enroll at the age of 65 to avoid late enrollment fees.
These are usually 10% of your monthly premium, and you will have to pay it for double the length of time you werent enrolled.
For example, if you enrolled in a Medicare plan at the age of 68 , you would have to pay the penalty for six years.
Why Would I Be Disenrolled From Medicare Plan B
Medicare may disenroll a person for non-payment of premiums. A person may also disenroll themselves by leaving the plan voluntarily.

To qualify for Medicare, individuals generally need to be 65 or older or have a qualifying disability. There are several levels of assistance an individual can receive as a dual eligible beneficiary. The term full dual eligible refers to individuals who are enrolled in Medicare and receive full Medicaid benefits.
Documents To Include With Form Ss
Once youve completed the form, youll need to gather the documents youre required to submit along with it to prove your identity, age, and citizenship statusthose of the person youre applying for a card on behalf ofor both. These documents typically include the following:
- An original birth certificate or other permissible proof of age, such as a passport, final adoption decree, or religious record of birth.
- An unexpired drivers license, passport, or state ID card to prove your identity, or if you dont have one of those, a military ID, employee ID, or another identity card.
- Additional evidence of citizenship, if you are not submitting a birth certificate or ID. If you arent a U.S. citizen, youll need evidence of immigration status.
The Social Security Administration will only accept originals or copies that are certified by the agency that issued the original. You cant use photocopies, even notarized ones.
If you plan to mail your documents to the Social Security Administration, get a certified copy of your drivers license from your state department of motor vehicles instead of sending the original.

Medicaid Renewals/recertifications In New York City
This article is for people whose Medicaid is administered by the NYC Medicaid program, run by the Human Resources Administration . If your Medicaid is handled by the New York State of Health your renewal is not covered in this article. The process is different outside NYC, though the general principles are the same.
Medicaid is authorized for one year at a time. Approximately 3 4 months before the end of your authorization period, you should receive a renewal packet by mail, which includes instructions on how to complete and return the packet to HRA. This process is also called recertification. Here are tips for handling renewals, including how to obtain a copy of the renewal form if you dont receive one in the mail.
Even if consumers opt to return their renewal during this period, they can attest to most aspects of eligibility and do not need to provide all the documents recommended below .

Change should be reported with Form 751k or in varioius languages here.
