If you are eligible for Medicare, the Texas’ Health Information, Counseling and Advocacy Program can help you enroll, find information and provide counseling about your options. This partnership between the Texas Health and Human Services system, Texas Legal Services Center and the Area Agencies on Aging trains and oversees certified benefits counselors across the state.
Those Age 65 Typically Get Premium
Medicare Part A is premium-free to those 65 and older who are also eligible for monthly Railroad Retirement Board or Social Security retirement payments because they worked and paid taxes during all 40 Quarters of Coverage.
This is one basis of eligibility for premium-free Medicare Part A. However, some people receive Social Security benefits before they turn 65.
The Social Security system automatically enrolls in Part A those individuals who receive Social Security or disability benefits for at least four months before the month of their 65th birthday. When this is the case, Part A coverage begins on the first day of their 65th birthday month.

The Individual Mandate And Late Enrollment Penalties
Although most states have eliminated the tax penalty set by the Affordable Care Act for not having health insurance coverage each year, Medicare has its own requirements for qualified health insurance.
Any of the below listed Medicare Parts will meet the requirements of the individual mandate for those age 65 and over:
- Original Medicare, consisting of Medicare Part A and Part B
- Medicare Part A has the Minimum Essential Coverage required by the Affordable Care Act.
- Medicare Part C Medicare Advantage plans meet the requirements of the individual mandate for qualified health insurance. These plans provide coverage equal to or better than Original Medicare.
Enrolling in Medicare Parts A and B on time is essential. For those who are eligible for Medicare, but who do not enroll, may face a penalty if they wait too long.

Prescription coverage is offered through private insurance companies either in the form of stand-alone Part D plans or all-in-one Medicare Advantage plans. Eligibility for Part D requires having either Part A or Part B.

Important member ID card changes for those with Medicare Advantage plans
The following changes begin on January 1, 2022:
- All Aetna Medicare Advantage plan members will have 12-digit member ID numbers beginning with 10. We will no longer use the ME prefix for Medicare Advantage plans after that date. Consol Energy will transition from ID numbers beginning with ME to those beginning with 10 on April 1, 2022.
- Group # will be known as Plan #.Please use the plan number in the way that you have previously used the group number in your transactions.
When checking your patients eligibility and benefits, use our provider portal on Availity or another vendor or clearinghouse on our list. Vendor fees may apply.
Note when services were performed:
- For services performed on or before December 31, 2021, use the members prior ID number.
- For services performed on or after January 1, 2022 , use the members 2022 MA plan ID number. You can use this number to verify coverage, request an authorization/precertification, issue referrals and submit claims.
Ask your patients for their current member ID card. If they dont have one, you can verify their eligibility using their full first and last name and date of birth. In addition, you can print an electronic copy of a patients ID card, if needed. Make sure that eligibility details match the patients information.

Who Is Not Eligible For Medicare Part A
Why might a person not be eligible for Medicare Part A? A person must be 65 or older to qualify for Medicare Part A. Unless they meet other requirements, such as a qualifying disability, they cannot get Medicare Part A benefits before this age. Some people may be 65 but ineligible for premium-free Medicare Part A.

How To Get Premium
The system of eligibility for Medicare measures the working years in which the individual paid the Federal Insurance Contributions Act tax, or FICA tax.
With the exceptions of self-employment and certain government positions, most people have this included in the payroll taxes deducted from each paycheck.
These working years accrue what is known as Quarters of Coverage.
Quarters of Coverage each represent a three-month period of work while paying the FICA tax about 4 Quarters of Coverage are earned in a single year.
The Quarters of Coverage are units of measuring eligibility for Medicare coverage and Social Security benefits. This measurement determines both the level of monthly Social Security income benefits and whether Part A is premium-free.
To get Part A premium-free, one typically must have earned 40 quarters of coverage, or about 10 years of employment while paying payroll taxes. The 40 total Quarters of Coverage do not have to be earned over 10 consecutive years.
These quarters of coverage can be earned through ones own work history or the work history of a spouse, parent, or child.
Those who have not quite accrued 40 Quarters of Coverage can still join Medicare Part A at age 65, but it will require paying a premium.
Earning 30-39 credits sets the monthly premium for Part A in 2021 at $259, and having earned less than 30 credits sets the Part A premium at $471.

When Should You Enroll For Medicare
Just because you qualify for something doesnt mean you need to sign up, right? Not always. In the case of Medicare, its actually better to sign up sooner rather than later. While its true that Medicare isnt mandatory, there are fees for signing up outside of your initial eligibility window. Also known as the initial enrollment period , this 7-month window gives you some flexibility in enrolling in Medicare once you qualify.
If you dont get automatic enrollment , then you must sign up for Medicare yourself, and you have seven full months to enroll. The IEP starts three months before the month you turn 65 and ends three months after the month you turn 65.
Initial Enrollment Period
Coverage start dates vary based on when you sign up. If you sign up during the first three months of your IEP , then your health insurance will take effect on the first day of your birthday month. If your birthday falls on the first of the month already, then your coverage will start on the first of the previous month. The longer you wait to sign up, the later your coverage will start, which is why its a good idea to check your eligibility and enroll as soon as youre able.
This initial signup period applies to people who dont get automatically enrolled. People who are already enrolled in Social Security and those with disabilities will be automatically enrolled into original Medicare depending on their situation.
Is Medicare Paid For By Original Medicare
Medicare services arent paid for by Original Medicare. Most Medicare Advantage Plans offer prescription drug coverage. or other. Medicare Health Plan. Generally, a plan offered by a private company that contracts with Medicare to provide Part A and Part B benefits to people with Medicare who enroll in the plan.

What About Medigap Plans
Original Medicare covers a good portion of your care, but its not exhaustive. Theres a wide range of services that Parts A and B dont cover, including dental and vision care. But if you like the flexibility of original Medicare and dont need the benefits that Advantage affords, but you still want additional coverage to offset your out-of-pocket costs, then consider adding a Medigap supplemental policy to your plan.
Medigap insurance, as its name suggests, covers some gaps left by original Medicare. Parts A and B require copays, coinsurance and deductibles, which can add up over time. And since Medicare requires Part B enrollees to cover 20 percent of their costs without any caps in place, you may face higher bills without some protection in place. Medigap offers that protection. With a supplemental plan in place, you may have coverage for:
- Copays and Coinsurance
- Deductibles for Part A, B or both
- Medical services while traveling
- Additional time in the hospital
This is just a sampling of the benefits that Medigap offers. There are 10 plan types, labeled A through N, and each plan type is standardized in all but three states .
