What Does Medicare Supplement Plan D Cover

Medicare Supplement Plan D pays many of the costs youreresponsible for with Original Medicare alone. Plan D covers the following expenses:

  • Medicare Part A coinsurance and hospital costs
  • Medicare Part B coinsurance and copayment
  • Foreign Travel emergency

Medicare Part A and Part B typically require you to pay 20% coinsurance. Theres no cap to how much you could have to pay out of pocket, but with Medigap Plan D, you wouldnt have to worry about it. As long as Medicare pays for a service, Plan D will pay the coinsurance, as well its portion of the other charges as charges listed above. A supplemental plan like Plan D can save you thousands of dollars in medical bills.

Foreign Travel Emergency Care

Medicare only covers foreign emergency health care in a small set of rare situations.

If you have Medicare Supplement Plans C, D, F, G, M, or N, your insurance company will pay 80% of the billed charges for certain medically necessary emergency care received outside of the United States.

You must pay a $250 deductible for these services. The plans have a lifetime coverage limit of $50,000.

Medigap Plan K And Plan L Have Annual Out

Plan K and Plan L each have an annual out-of-pocket spending limit.

Once you reach this limit within a calendar year, the plan will pay 100 percent of the costs for your covered Medicare services for the remainder of the year.

The Plan K out-of-pocket maximum is $6,620 in 2022. The 2022 Plan L out-of-pocket spending limit is $3,310.

The recent acquisitions of Aetna by CVS Health and Express Scripts by Cigna have resulted in further consolidation of the Part D marketplace. In particular, between 2018 and 2019, Cigna increased its market share from 3 percent to 8 percent, while CVS Health increased its market share from 14 percent to 17 percent. Under the CVS Health-Aetna merger, Aetna divested its stand-alone PDP business to WellCare, resulting in a more than doubling of WellCares Part D market share, from 4 percent in 2018 to 10 percent in 2019.

Medicare Supplement Insurance: Plan D

Medicare Supplement insurance plans may help with certain costs not covered by Original Medicare. There are 10 standardized plan types available, each labeled with a different letter. Each plan type corresponds with a different level of coverage and works alongside your Original Medicare, Part A and Part B, benefits.

Among the 10 Medigap plan types, Medigap Plan D might be viewed as the midpoint in terms of the amount of coverage. Plan D offers more coverage than Plans A and B, but does not offer the additional benefits covered by Plans C and F. Certain benefits included in other plans, such as the Medicare Part B deductible and Part B excess charges, are not covered under Plan D.

Please note that âPlan Dâ should not be confused with Medicare Part D, which is Medicare prescription drug coverage. Plan D is supplementary insurance that fills some coverage gaps in Original Medicare, while Part D is the Medicare prescription drug benefit, available through Medicare Prescription Drug Plans and Medicare Advantage Prescription Drug plans.

What Does It Mean If My Prescription Drug Has A Requirement Or Limit

Medicare Supplement Plan N vs Plan G

Plans have rules that limit how and when they cover certain drugs. These rules are called requirements or limits. You need to follow the rules to avoid paying the full cost of the drug out-of-pocket. If you do not get approval from the plan for a drug with a requirement or limit before using it, you may be responsible for paying the full cost of the drug. If needed, you and your doctor can also ask the plan for an exception.

Here are the requirements and limits you may see on a drug list:

PA Prior Authorization

If a plan requires you or your doctor to get prior approval for a drug, it means the plan needs more information from your doctor to make sure the drug is being used and covered correctly by Medicare for your medical condition. Certain drugs may be covered by either Medicare Part B or Medicare Part D depending on how they are used. If you don’t get prior approval, the plan may not cover the drug.

QL Quantity Limits

The plan will cover only a certain amount of a drug for one copay or over a certain number of days.

ST Step Therapy

The plan wants you to try one or more lower-cost alternative drugs before it will cover the drug that costs more.

B/D Medicare Part B or Medicare Part D Coverage Determination

Depending on how they’re used, some drugs may be covered by either Medicare Part B or Medicare Part D . The plan needs more information about how a drug will be used to make sure it’s correctly covered by Medicare.

What Does Plan D Not Cover

There are just two benefit areas where Plan D does not offer any coverage.

  • The Medicare Part B deductibleYou must first satisfy an annual deductible before your Medicare Part B medical insurance kicks in. In 2022, the deductible is $233 for the year. That means you must pay for the first $233 worth of your Part B covered services and items out of your own pocket. Part B coverage then kicks in once that deductible has been reached.
  • Medicare Part B excess chargesSome health care providers don’t accept Medicare assignment, which means that they don’t accept Medicare reimbursement as payment in full for their services or items. These providers reserve the right to charge you up to 15% more than the Medicare-approved amount for their services or items. These charges are called Medicare Part B excess charges.

If you are enrolled in Medicare Part A and you want to sign up for Part B, please complete form CMS-40B, Application for Enrollment in Medicare Part B . If you are applying for Medicare Part B due to a loss of employment or group health coverage, you will also need to complete form CMS-L564, Request for Employment Information.

You can use one of the following options to submit your enrollment request under the Special Enrollment Period:

  • Fax or mail your CMS-40B, CMS-L564, and secondary evidence to your local Social Security office .
  • Note: When completing the forms CMS-40B and CMS-L564:

    • State I want Part B coverage to begin in the remarks section of the CMS-40B form or online application.
    • If possible, your employer should complete Section B.
    • If your employer is unable to complete Section B, please complete that portion as best as you can on their behalf and submit one of the following forms of secondary evidence:
      • Income tax form that shows health insurance premiums paid.
      • W-2s reflecting pre-tax medical contributions.
      • Pay stubs that reflect health insurance premium deductions.
      • Health insurance cards with a policy effective date.
      • Explanations of benefits paid by the GHP or LGHP.
      • Statements or receipts that reflect payment of health insurance premiums.

    A Hospital Care Coinsurance

    Medicare Part A covers inpatient hospital stays. But if your stay lasts longer than 60 days, you have to make a coinsurance payment for each day youre still in the hospital.

    After a certain point, Medicare stops covering your inpatient hospital costs altogether.

    If you have a Medicare Supplement insurance policy, it will cover your Part A coinsurance costs.

    • The coinsurance is $389 per day in 2022 for days 61 to 90 of your hospital stay.
    • Beyond 90 days, theres a $778 per day coinsurance cost in 2022.

    If you have a Medigap policy, it also will cover any inpatient hospital costs after Medicare stops covering them. It will do so for up to 365 days after you use up your Medicare benefits.
